For years, Medicare has been legally prohibited from covering prescription drugs used purely for weight loss. That is about to change. The Centers for Medicare and Medicaid Services (CMS) has announced two overlapping programs that will give eligible Florida seniors access to GLP-1 medications like Wegovy and Zepbound at dramatically reduced cost, starting as early as July 2026.

CMS has contracted with Humana as the central processor, which means that prior authorizations, claims, and pharmacy payments all run through Humana regardless of which Medicare plan you carry.

2. The BALANCE Model (Medicaid: May 2026 / Medicare Part D: January 2027)

BALANCE stands for Better Approaches to Lifestyle and Nutrition for Comprehensive hEalth. It is a longer-term voluntary model in which CMS negotiated directly with manufacturers Eli Lilly and Novo Nordisk on pricing.

For Medicaid enrollees, the BALANCE Model is already launching in participating states as early as May 2026. For Medicare Part D, the BALANCE Model is structured to begin January 1, 2027, though CMS has confirmed that the 80% plan participation threshold required to trigger the full launch was not met, making the GLP-1 Bridge the primary Medicare access pathway through the end of 2027.

Who Qualifies for the GLP-1 Bridge?

Eligibility is clinical, not automatic. Your provider must submit a prior authorization request attesting that you meet one of the following criteria:

  • BMI of 35 or higher at the time GLP-1 therapy was initiated
  • BMI of 30 or higher plus one qualifying condition: heart failure with preserved ejection fraction, uncontrolled hypertension, or chronic kidney disease stage 3a or higher
  • BMI of 27 or higher plus one qualifying condition: pre-diabetes, previous heart attack, or previous stroke

You must also be enrolled in a standalone Medicare Part D plan or a Medicare Advantage plan with prescription drug coverage (MA-PD). Special Needs Plans and employer group waiver plans are also eligible.

Dual-eligible beneficiaries, those who qualify for both Medicare and Medicaid, may have additional pathways through the BALANCE Medicaid launch.

The clinical criteria are worth examining closely because they map directly onto conditions that are common in the senior population. One important caveat: the prior authorization process requires your provider to submit to the Bridge's central processor through a specific billing code (BIN 028918 / PCN MEDDGLP1BR), not to your regular insurance company. Providers unfamiliar with the Bridge may need guidance. It may be worth printing the CMS overview and bringing it to your next appointment.
